The change you've made was not the one I wanted.  There are already
several adjacent entries in the index from 'cadenza' and 'unmetered',
but now there are none from 'bar lines' and 'bar numbers'.  There should
be.  If you don't like 'turning off', how about simply
@cindex bar lines, cadenzas
@cindex bar numbers, cadenzas
ditto for unmetered music
to go with the ones for beams and accidentals?
