Limit on page size

There is no way, because page sizes must be representable as Lout
lengths, and these are 16-bit quantities whose maximum value is 57c.

The only possibility is to produce pages within this limit and to use
some PostScript postprocessor which scales the output.  It's not hard
to do: something like

   2.0 2.0 scale

stuck into the PostScript at just the right spot near the beginning
of each page would do the trick.
