hyphenation woes

I think I've fixed those hyphenation problems.  In the course of
working it all out I concluded that the bugs were confined to
manually inserted hyphenation points (i.e. &-), not automatic
ones.

Following TeX, Lout makes two attempts to break each paragraph.
On the first attempt, hyphenation is disabled.  If the final
quality is sufficiently good that's the end.  Otherwise,
hyphenation is enabled, hyphenation points are calculated for
all words and inserted into them, and the pragraph is broken
again.

The difference between manual and automatic hyphenation points
is that the manual ones are present in the paragraph on both
attempts, whereas the automatic ones are only inserted just
before the second attempt.  That's why it is possible to have
a bug that affects manual hyphenation points but not automatic
ones.

Anyway, I don't suppose anyone cares about these details.

The next release will have the fix.
