Lout 3.12 generates completey resovled doc in one pass.

Ghostscript-5.50 handles the lout generated PostScript without error.

My HP 4000N printer prints the PostScript WITHOUT any second page or
"undefine".

My operating system is Solaris 2.5.1. I'm printing via the standard
Solaris printer system using HPs jetadmin filters. These filters
simply wrap in incoming PostScript in PJL start and end headers. They
do NO convert the PostScript to PCL.

Using ghostscript to convert the PostScript to PCL and printing it
also shows no problems. The printer's name is oak and hpnpf is part of
the HP jetadmin suite. hpnpf eventually gets called by the lp system
but using it as shown below bypasses the lp system completely.

    gs -sDEVICE=ljet4 -sOutputFile=test.pcl -dNOPAUSE -q test.ps -c quit
    /opt/hpnp/bin/hpnpf -x oak test.pcl

My guess is the problem is not directly related to lout. What
operating system are you using? What filters (if any) are in you
lp/lpr system? Are the filters different from the Apple Laserwriter
setup compared with the HP 4000N setup.
