Having a column for margin notes under the running headers is quite
feasible.  Whether I will get around to it any time soon is another
matter; I've just given more time than I had available to getting
Perl formatting into Lout.

I suspect that page layouts varying with the chapter are virtually
impossible to add to the current package.  It would need to be
completely redesigned along different lines.

In principle, there is nothing to prevent you making your own
packages that do these things.  If it's for work, maybe they
can pay someone to do it.
