I wanted at my favourite booklet issue format to display the cover and the back
cover alone, inside of the booklet to display even-odd pages in sheets, and the
written page numbers follow the original pages instead of sheets' numbers. For
these jobs I was created a patch (rfxview_2p.swf), Check changes in source code
(rfxview_2p.sc) signed by 'BI':

Usage:
At first create a one-sided pdf file (blank.pdf) and than put it into your pdf
document (file.pdf)
- before the cover,
- and also after the back cover, so file.pdf has 2 pages more from now.
Tip: there are a lot of different ways to insert page, eg. Acrobat Pro (mac or
pc), PDFEdit, scribus (linux) or http://www.pdfescape.com/ online or by
serverside applications (php, perl, python, etc.).
Tip: give blank page the same color as rfxview (default: #CCCCCC).
At the end convert your pdf to two-pages-swf from command line:
# pdf2swf -B rfxview_2p.swf -o file.swf -2 file.pdf
