Mmmmh... It may well be that I forgot an important step in the
manual: Perhaps it is necessary to run "texhash" (as root if you've
installed texi2latex in /usr/) immediately after having installed
texi2latex. "texhash" assures that all TeX-related programs find
their files.
By the way, on most Linux systems, "texhash" is called regularly
(e.g. daily). So it could be that it works tomorrow ... ;-)

I think I just forgot it. However, I read the source of index.sty
and apparently, \makeindex just sets up the default index. Since
texi2latex has no default index (every index command has an index
class in brackets [...]), it should be pointless to call it anyway.
