I thought a bit about whether the remaining of the  menus should 
be in code_style or not, but I don't have a definitive answer. The 
fact that the user should be assured that his formatting is 
conserved would tend to having everything in code_style. In the 
manual there is

"Space characters in a menu
are preserved as-is; this allows you to format the menu as you wish."

However in some formats (html, xml) it may be desirable not to keep the 
spaces as is and lose some user formatting. So, my current thought about 
it is:

* if the spaces are conserved, format all the menu in code_style if
  possible
* otherwise there is no rule

I think that in xml it should be in code_style in menus, and also in
info, but not necessarily in html.

It seems to me that therse rules should also be followed in menu comment
lines, although it is not always easy to know when one is in a menu
comment, especially when there are some block formats in menu
descriptions or in menu comments.
