Switched capacitor circuits are horribly inefficient, noisy, have poor regulation and output voltage granularity. The only part of the circuit that limits currents are ESL and ESR in the switch.http://www.geofex.com/circuits/+9_to_33.htm 9v to 33v isn't to far off. At first glance I don't see what would stop you from changing all those D's and C's to a zener and a single C. Looks like it does about .2mA a bit shy of the 2mA max for the 4702 or the 4707. Looks like there are a number of chips from Maxim-ic.com could work fairly well.
If you need 2mA at 33V, you could probably get away with it, but it's quite a hack. Inductors aren't really anything to be afraid of... :)
