Actually, it creates the signature just fine and then (sometimes,
depending on the nature of your filesystem) immediately deletes it
while pruning the ,,inode-sigs dir of old signatures.   It mistakes
the new signature for an old one.

It's a flaky that I have been unable to reproduce but last night
jblack, asuffield, and rbtcollins who saw it more consistently worked
on it on IRC, nailed it, explained it and every observed symptom of it
perfectly, observed it happen via strace, etc. -- there's a fix
waiting for me to merge in (later today).
