It only works nicely on account of the content of your first column (HCT, 
Metoprolol) being so few in characters. Try

        TESTOSTERONE CYPIONATE
        CIPROFLOXACIN HYDROCHLORIDE

… however I had been unable to recognize the need to support a fourth column 
whose rows may (or may not) have content.

It would be clearer to add a column heading 'Advice'

        Substance & Strength & Brand & Advice \\

but there will be insufficient page width. IMO better to do:

        Substance & Strength & Advice \\
        Brand

and return the Brand in alternating rows (or, of no brand, just ''& ) and put 
an hline after every second row.
