[Gnumed-devel] vaccination edit are nearly finished

Reworked vaccination panel focussed on indications rather than
regimes. After all, we vaccinate for indications (but check
against recommended courses to find out which shots are
"missing" - from the recommended set of courses, that is).

It now allows to edit an old entry as well as entering new
ones complete with phrase wheel support. The missing
vaccinations panel is not up to par yet. The widget listens to
the database for changes made to the patient data.
