Here's an answer to one of the questions raised there:

Why the gnumed shell wrapper and why so complex ?

1) we have reduced the complexity to the absolute
   minimum in 0.4

2) the reason for it to exist is that it allows for admin
   and user to define local extension shell scripts running
   other things before actually starting GNUmed
