Certainly, I'm not saying it doesn't happen, or that it shouldn't
happen, or that there aren't things that could be changed to make it
more convenient.  

You wouldn't necessarily have to keep things disconnected, of course,
provided everyone uses unique branch names, everything should be OK.
For convenience a consultant might well choose to keep one or more
databases for each client, of course.
