Looks good. I'm gonna merge it in.
One thing to be aware of is that Gfx::Gfx (the part of xpdf / Poppler
that calls startPage) will also draw a clipping rectangle onto
the page afterwards. For an unpatched Poppler codebase, that'll need
to get ignored by GfxOutputDev/BitmapOutputDev somehow.

How are you going to access the private functions pdf2swf
currently uses, e.g. SplashOutputDev::doUpdateFont?
