It seems to me that this is an old or strange merged version since there 
are 15.943 "msgctxt" found (corresponding to this amount of ICD codes).

E.g. there are two entries for E110:

one outcommented:

#~ msgctxt "model:gnuhealth.pathology,name:E110"
#~ msgid "With coma"
#~ msgstr ""
#~ "Nicht primär insulinabhängiger Diabetes mellitus [Typ-II-Diabetes]: 
Mit "
#~ "Koma"

one with missing german translation:
msgctxt "model:gnuhealth.pathology,name:E110"
msgid "Type 2 diabetes mellitus: With coma"
msgstr ""

But they should be merged like:

msgctxt "model:gnuhealth.pathology,name:E110"
msgid "Type 2 diabetes mellitus: With coma"
msgstr "Nicht primär insulinabhängiger Diabetes mellitus 
[Typ-II-Diabetes]: Mit Koma"

If possible, it would be easier to follow if there are no outcommented 
entries, I think.
