About HTML output.

About formatting of the HTML output -- as you may already know, since
rms first invented Texinfo, it has never been a goal to have
author-customizable output.  In fact, it's explicitly not a goal, so
that authors aren't tempted to spend their time fiddling with the
formatting instead of working on the document.  The HTML CSS stuff is an
exception in that regard.

The beginnings of documents are always troublesome.  I don't claim that
the current HTML output has the perfect frontmatter, in fact I'm sure it
isn't.  I simply lack a concrete idea for how to improve it, given all
the different scenarios that Texinfo tries to support.

About the (lack of) the DTD declaration -- I am a lot less interested in
strictly adhering to standards than I am in writing HTML that works on
as many browsers as possible.  I'm not aware that the lack of a DTD
actually causes a failure in any browser (as opposed to merely failing
to validate).  If you are, please inform.
